Whatever country, province, state or region that you live in, one of the first organizations you should contact is your local Hospice. Hospice is international and is a great place for you to start reaching out for information, guidance and help following a terminal diagnosis, as well as Health Organizations such as the Canadian/American Cancer Society (or your afflicted loved ones disease organization if not cancer), and the Ministry of Health.

And so importantly, gently encourage your terminally ill loved one to ask their family doctor or specialist for a referral to a Palliative Care Physician; and for Ontario residents, contact their local Community Care Access Centre (CCAC) in addition to the above.
